ProTaper Gold’s patented advanced metallurgy produces a distinct difference that the clinician can feel and feel. ProTaper Gold Tackles for Narrow Canals

Case Summary

After identifying tooth #19 as having necrotic pulp as well as chronic abscess on the apical side I suggested RCT for an elderly woman aged 66. The main challenges with this case were finding a path that allows for a glide route to the working length, canals that are calcified and a curvature that was a bit tricky. Diagnosis

This case involves a 66 year old female with a lengthy medical background. The patient has a history of High Blood Pressure, High Cholesterol anxiety, and the recent discoveries of Melanoma as well as Breast Cancer. She has had a past history of chemotherapy and radiation treatment. She is currently taking Anastrozole, Zocor, Lexapro and HCTZ. There have also been blood transfusions, and has an allergy to levoquin.

Her main complaint was that she felt that the “dentist observed something on the xray however, I didn’t feel any pain at all.” After she was finished with the majority of her treatment for cancer she could manage the oral infection.

The radiograph shows tooth #19 being restored using an amalgam filling. Its PDL is thickened, and the lamina dura appears diffuse. There is a significant periapical radiolucency that is associated the tooth number 19. The radiograph also shows apical calcium.

Clinically speaking, the tooth has sinus tract that drains. The tooth was found to have normal probing depths, there was no discomfort from percussion, and no reaction to cold.

Dental tooth diagnosed as having Necrotic Abcess and Pulp. It was also diagnosed as Chronic.
Access and Shaping

The entrance was easy however there was a pulp stone within the chamber.

The canals were 22mm long , and finding my glide path to work length was perhaps the most difficult problem in this instance. In the beginning, I employed C file and EDTA to get an even patency. I utilized my handpiece from the endo together to the C files to become more efficient in the procedure.

The other issue was curvature of the MB root. The combination of curvature and calibration can result in an unorganized file, which is why I prefer to give this portion of the process more attention.

Once the glide path had been established, I felt confident I had my motor set to go. My preferred choice is ProTaper Gold, especially in the case that has a curvature that is similar to this one.I am confident that it can go through the bend easily. This kind of metal has greater flexibility and performs better in my hands because I don’t feel as many resistance. In the course of the procedure I made sure to use a steady stream of sodium hypochlorite 6. If I came across any calcification, I changed to 17 percent EDTA to ease the way through any obstruction. Once the instrumentation was complete I did the final flush using 5cc of 17 percent EDTA along with 5cc sodium hypochlorite.

My master apical record was an 25/.04 in the mesial canals, and a 30/.04 on the canals that were distal. There were four canals altogether.

I utilize a hybrid approach of S1, F2, and then change to ProFile 25/.04 and 30/.04.
Obturation and Irrigation

In this instance I didn’t activate my irrigants as I do not consider them to be effective in situations with narrow canals. However, I do prefer to use a 30 gauge ProRinse needle for my irrigation, so that I get the maximum needle penetration to ensure more efficient irrigation.

I removed the tooth using hot vertical condensation. Then I sealed the tooth with ThermaSeal’s the Ribbon Plus Sealer.

Restoration

The flowable material was put over each orifice for an orifice-blocking material and a cavit and sponge were put in. The patient was then referred again to the GP for the final reconstruction.

Things to Remember:

In this kind of situation the most important thing to remember is to establish your path of travel so that you are secure using your rotary file. Don’t expect your rotary files to do miracles, but instead learn to take take care of it just as any other item. The rotary files aren’t foolproof however if you take the time to provide your rotary precisely what it requires it will make your life much easier and help keep your treatment secure.
